Additional Documentation

In order to comply with Federal regulations the Office of Student Financial Services may ask FAFSA contributors to provide information that verifies the data provided on the FAFSA. This information could include, but is not limited to, using the a signed copy of the Federal 1040 tax form and schedules filed, your parents’ signed copy of the Federal 1040 tax form and schedules filed, an official tax return transcript*, your parents’ official tax return transcript*, and/or independent or dependent verification forms.

Required additional information will be requested through the student’s MaineStreet To Do List and will include detailed instructions and links to specific forms. It is very important to respond to these requests by our document due date and follow instructions closely otherwise students may be considered a late filer and financial aid eligibility could be affected.

Students and contributors do not need to send us supporting documents unless we request them specifically.

It is very important to fill out all required forms completely (leave no field blank) and include the Student ID on all documents. Failure to follow instructions carefully will delay processing.

If a FAFSA contributor is ineligible to use the Direct Data Exchange (FA-DDX) and must submit a tax return transcript to our office, click here for specific directions on requesting a tax return transcript.

** FAFSA contributors that filed a joint tax return and are separated/divorced should contact our office for further instructions**
