Greater Portland Health (GPH) is a Federally Qualified Health Center (FQHC) that was established in 2009 to provide high-quality patient-centered health care to individuals and families in the Portland community, regardless of ability to pay. Now with nine locations across Portland and South Portland, Greater Portland Health partners with specialists, hospitals, pharmacies and other area providers to deliver accessible, affordable and culturally sensitive care to those in need. Greater Portland Health is looking for a Licensed Clinical Social Worker, Licensed Alcohol and Drug Counselor to provide integrated substance use and mental health services in a community health setting. The social worker will be responsible for performing biopsychosocial assessments, developing individualized treatment plans, providing individual and group counseling and crisis intervention to patients experiencing homelessness. This person will be an integral member of an inter-disciplinary health care team and will work closely with medical providers, community health workers, nurses and peer support specialists to support patients’ emotional health and well being.
Experience working with individuals experiencing homelessness, substance use treatment, and dual diagnosis required.
