-Address behavioral health needs through solution-focused, short-term interventions- -Work alongside medical providers in a primary care setting utilizing a shared EMR- -Serve people of all ages and economic situations in a family practice environment- -Empower people in various stages of the life cycle to take control of their health- -Participate in the larger team of BHCs serving a network of 11 family practices-

Compensation: Competitive salary and benefit package

-Develop a common treatment plan addressing physical and emotional needs- -Assist people with chronic diseases in learning new coping strategies- -Work 4-day weeks with 6-9 scheduled patient visits per day-

Sign-On Bonus: $5,000 – $7,500

Requirements: Independent clinical social work license is required. LADC is also beneficial.

A private, non-profit with a 45-year history, HealthReach is funded by patient fees, grants and individual donations.
