Several Positions at Community Clinical Services

Community Clinical Services is currently hiring for several positions in our Outpatient Behavioral Departments. These include: two Adult Case Managers, one Child Case Manager, an integrated Clinician and two Front Office positions:  Position: Social Worker BS Level (Behavioral Case Management Department) Community Clinical Services, affiliated with St. Mary’s Health System, is seeking three organized, self-motivated individuals to provide case management services to clients with emotional or psychiatric diagnoses as part of the Behavioral Health Home Organization. Our program continues to expand and, as a result, have need to hire two Adult Case Managers and a Child Case Manager. The BHHO is a team approach to case management and provides improved care coordination and access to services that address both mental and physical health, as a holistic approach to wellness. This person will function as part of an interdisciplinary team and will be responsible for managing a caseload of patients.

Responsibilities:

✓Provides community case management services for specified client population (i.e. adults or children/adolescents)

✓Collaborates with other providers to provide holistic care to clients

✓Documents biopsychosocial assessment, progress notes and treatment goals in an Electronic Medical Record (EMR)

✓Tracks and manages quality care initiatives

✓Assists with authorization and attestation process for services rendered in Atrezzo portal

Education and Requirements:

•Must be eligible to be licensed or eligible to hold certification in the State of Maine (LSW or LSX for Children or MHRT/C for Adult)

•1-3 years’ experience in case management preferred, but not required

Position: Clinician II (Outpatient Counseling Department  )This position will provide counseling services in an integrated primary care setting and our outpatient clinic (Located in Poland, Lewiston, and Auburn). This is a year-round, salaried and benefit eligible position. This role will be responsible for collaborating with other providers to provide holistic care to clients. Brief, solution focused therapy is the indicated therapeutic model for our integrated primary care sites.In our outpatient clinic setting, we offer long term counseling services.The ideal candidate will model appropriate social interaction skills, including boundaries, healthy communication, conflict resolution etc. with patients, families, school personnel, peers and others.

Responsibilities:

✓Provides long term counseling in an outpatient clinic setting

✓Provides short term, solution focused treatment to clients in a primary care setting

✓Collaborates with other providers to provide holistic care to clients

✓Documents client assessment, progress notes and treatment goals in an Electronic Medical Record (EMR)

Education and Requirements:

•Master’s Degree in Social Work required

•Must be licensed as a Clinical Social Worker in the State of Maine (LCSW)

•1-3 years experience in an outpatient and/or school setting preferred
