From the job ad: “The Health Home Coordinator will provide care coordination/case management to children who have a DHHS approved mental health or intellectual disabilities diagnosis.  In the BHHO model, you will participate in a team-based approach to care, by working in collaboration with a Clinical Team Leader, Family/Youth Support, Nurse Care Manager, Psychiatric and Medical Consultants. As a Health Home Coordinator, you will work with children and their families to address behavioral health needs, as well as integrating physical health and medical needs. You will work with a diverse population of families.  Candidate should possess excellent communication, interpersonal skills, documentation skills and the ability to multitask.”
