Behavioral Health Services looking for an HCT Clinician

SCOPE: The HCT Clinician provide Behavioral Health Services to children and families in the home and community settings. The primary focus of the Clinician is to develop and implement an Individual Treatment Plan for each client and supervise the Behavioral Health population. The goal is to increase the child’s level of function and skill development and to decrease maladaptive behaviors.
RESPONSIBILITES:
· Work with referring agencies, parents and children on the development of an Individual Treatment Plan (ITP).
· Implementation, evaluation, and adjustment of ITP.
· Opens all cases in a timely manner and works with the Program Director to assign Behavioral Health Professional. Supervision of employees in group and/or in a one on one setting.
· Problem solving with families related to ITP and meeting goals.
· Crisis planning and management with family, BHP employee, and case manager.
· Adhere to the Department of Behavioral and Developmental Services (BDS) guidelines and regulations. This is to include the conducting of assigned trainings to meet BDS requirements for staff.
· Attendance of weekly meetings with team to report the status of the implementation of the goals and objectives established by each client’s ITP.
· Maintains effective relationships with case managers
. · Facilitate and maintain the appropriate paperwork in a timely manner, as established by the Policies and Procedures put forth by SRHS.
· Maintains professional licensure in accordance with appropriate standards.
EDUCATION: The Clinician must meet the guidelines established by the Department of Health and Human Services. Clinicians must be LMSW-CC or LCSW level.
Job Type: Full-time
Pay: $40.00 – $45.00 per hour
