Social Worker–CHANS Hospice Maine Health–Mid Coast–Parkview Brunswick, ME Full Time–Days Mid Coast –Parkview Health is seeking a Social Worker in Brunswick. Social Worker positions at CHANS Home Health and Hospice are medical social services positions. Hospice Social Workers collaborate with all members of the Hospice interdisciplinary team to enhance understanding of the significant social & emotional factors related to the patient’s health problems, and to identify & facilitate solutions. The overall goal of Social Work services is to improve or maintain the social, emotional, functional, and physical health status of the patient, and to ensure that the patient’s needs are being met in the home environment rather than resorting to premature or unnecessary institutionalization.
Qualifications:
•Master’s degree from an accredited college preferred.
•Maine State Social Work License (LCSW).
•One–year minimum social work experience in related area (Hospice).
•Community health experience preferred.
•Prior keyboard/typing experience for efficient use of laptop when inputting patient data at Point of Care.
•Completion of a four–hour orientation specific to hospice service preferred.
•Those who are hospice certified must obtain 8 hours of continuing education annually.
