OHI is seeking a Community Case Manager (CCM) to join our enthusiastic Team to provide services to adults with Intellectual Disabilities.
Our CCM’s
- Have the determination to empower individuals to lead self directed lives and enhance involvement in their communities.
- Will be a visionary and establish a course of action for self and others to prioritize goals to meet all deadlines.
- Create and assess service plans.
- Will be an integral part of the team and maintain open lines of communication
The successful applicant will have:
- Bachelor’s Degree in Mental Health Related Field
- Maine Driver’s License and reliable transportation
Job Type: Full-time
Pay: $15.00 – $15.25 per hour
