-Assist people of all ages and economic situations in a family practice outpatient setting
-Develop
treatment plans with medical clinicians addressing physical and emotional needs
-Utilize solution focused, short-term interventions
-Collaborate with BHCs in a network of 11 sites

~Franklin County Compensation:
•Competitive salary and benefit package
•Sign-On Bonus: $5,000-$7,500
•Student Loan Repayment opportunities
•Paid Relocation
•Continuing Education: $2,500 ~ 1 week per year
•PTO–272 Hours Annually
•Retirement Plan with employer match up to 2% of salary
•Consultation towards LADC licensure
•Malpractice Coverage
•4-days week; 6-9 scheduled patient visits per day
Requirements: Independent clinical social work license is required. LADC is also
beneficial.

In addition, a sliding fee program is available to uninsured and underinsured residents as well as assistance with applications for programs that help with the cost of health care and medications including the Health Insurance Marketplace. A private, non-profit with a 45-year history, HealthReach is funded by patient fees, grants and individual donations.
