Responsibilities:
-Address behavioral health needs through solution-focused, short-term interventions- -Work alongside medical providers in a primary care setting utilizing a shared EMR- -Serve people of all ages and economic situations in a family practice environment- -Empower people in various stages of the life cycle to take control of their health- -Participate in the larger team of BHCs serving a network of 11 family practices-
Compensation: Competitive salary and benefit package
-Develop a common treatment plan addressing physical and emotional needs- -Assist people with chronic diseases in learning new coping strategies- -Work 4-day weeks with 6-9 scheduled patient visits per day-
Sign-On Bonus: $5,000 – $7,500

Continuing Education: $2,500 ~ 1 week per year PTO – 272 Hours Annually

In addition, a sliding fee program is available to uninsured and underinsured residents as well as assistance with applications for programs that help with the cost of health care and medications including the Health Insurance Marketplace. A private, non-profit with a 45-year history, HealthReach is funded by patient fees, grants and individual donations.
