Two positions at St. Mary’s Health System

St. Mary’s Health System, a member of Covenant Health, is seeking candidates to join our expanding Behavioral Health Home team at Community Clinical Services! We are currently looking to add new positions within our team due to the growth of our BHH program. Currently, we are seeking 2 CHILD CASE MANAGERS, one ADULT CASE MANAGER, as well as, a PEER SUPPORT SPECIALIST and a FAMILY SUPPORT SPECIALIST to serve our population of clients.
Position: Social Worker BS Level (Behavioral Case Management Department)
Community Clinical Services, affiliated with St. Mary’s Health System, is seeking an organized, self-motivated person to provide case management services to clients with emotional or psychiatric diagnoses as part of the Behavioral Health Home Organization. The BHHO is a team approach to case management and provides improved care coordination and access to services that address both mental and physical health, as a whole person approach to wellness. This person will function as part of an interdisciplinary team and will be responsible for managing a caseload of patients.
Responsibilities:
Provides community case management services for specified client population (i.e. adults or children/adolescents)
Collaborates with other providers to provide holistic care to clients
Documents biopsychosocial assessment, progress notes and treatment goals in an Electronic Medical Record (EMR)
Tracks and manages quality care initiatives
Assists with authorization and attestation process for services rendered in Atrezzo portal

Education and Requirements:
Must be licensed or hold certification in the State of Maine (LSW or LSX for Children/ MHRTC for Adult)
1-3 years’ experience in case management preferred, but not required
Ability to work in a team setting and autonomously
Excellent written, verbal and interpersonal skills required

Position: Peer Support Specialist (Behavioral Case Management Department)
Community Clinical Services, affiliated with St. Mary’s Health System, is seeking two peer support specialists (one Adult Peer Support Specialist and a Family Peer Support Specialist) to provide services on a part time basis to clients with emotional or psychiatric diagnoses as part of the Behavioral Health Home Organization. Both positions are expected to expand to full time roles by 2020. The BHHO is a team approach to case management and provides improved care coordination and access to services that address both mental and physical health, as a whole person approach to wellness. This person will function as part of an interdisciplinary team and will be responsible for engaging with clients through one-on-one meetings, facilitation of peer support groups, education, and outreach.
Responsibilities:
Provides peer support services for specified client population (i.e. children/adolescents), as follows:
  • Models the attributes of respect, trust, empathy and confidentiality to clientele, colleagues and community providers.
  • Models healthy relationships.
  • Assures that members receive timely, coordinated services/ support that address physical and behavioral health needs, and promote community and home-based recovery.
  • Offers support during transitions, as needed/desired by the member.
  • Collaborates with Individual, Family Supports and providers
  • Offers assistance with navigating the mental and medical health system of care
  • Offers guidance and coaching on self-advocacy techniques
  • Engages clientele and their families in connecting to wellness/recovery opportunities in the community and accessing other community support services when appropriate.
  • Demonstrates ability to utilize strengths-based approach to problem solving challenges
  • Ability to work a flexible work schedule
  • Good communication skills and an ability to establish rapport with consumers, family members, caregivers and other resource providers.
Documents progress notes in an Electronic Medical Record (EMR)

Education and Requirements:
High school diploma or equivalent required; some college preferred.
Valid Maine state driver’s license.
State of Maine Certified Intentional Peer Support Specialist (CIPSS) Certification preferred or Provisional CIPSS Certification within Nine (9) months of hire date. (Employment will be contingent on the above certification guidelines.)
1-3 years’ experience in peer support role preferred, but not required
Ability to work in a team setting and autonomously
Excellent written, verbal and interpersonal skills required

Experience:
Self-identified mental health consumer of care.
