EIM is Maine’s leader in care coordination, connecting people throughout Maine with extensive home care services. We work with consumers, families, and health care professionals to provide consumers with the best services available in their community.We have full-time openings in Lewiston and Bridgton/Naples. EIM Care Coordinators assist consumers participating in Long Term Care Services and Supports through person-centered, strength-based, proactive planning using a collaborative partnership approach, promoting self-determination through advocacy, self-care management, and use of evidence-based practices while following applicable policies and procedures. Care Coordinators perform face-to-face consumer visits; complete assessments; and develop goals, actions plans, and backup plans to meet the consumer’s stay-at-home needs.
