We are currently seeking qualified candidates to join our team in the following positions:
Social Worker II (Licensed at the Master level by the State Board of Social Work licensing required.)
MaineGeneral Home Care, Skowhegan, ME 32-40hrs.
The Hospice & Homecare Social Worker provides emotional support to individuals and families in all phases of illness, including end of life. Social Work support is provided in whatever setting the patient and family call home. The social worker may act as a preceptor to new social work staff.
Responsibilities: ​
·       Provides appropriate information/education to patients/caregivers including
patient/caregiver rights and responsibilities.
·       Addresses psychosocial needs in a holistic manner in collaboration with the patient,
caregiver, and IDT.
·       Has knowledge of community mental health system and services.
·       Performs thorough and accurate pre-bereavement and bereavement assessments.
·       Is an engaged and collegial member of the IDT.
·       Has knowledge of effective crisis intervention
The ideal candidate will have:
·       Licensed at the Master level by the State Board of Social Work licensing required.
·       Maine Driver’s License required
·       Master’s degree in Social Work required
·       1 year or more of pertinent experience preferred
