Behavioral Health Home Team
St. Mary’s Health System, a member of Covenant Health, is seeking two full-time, salaried Case Managers to join our expanding Behavioral Health Home team at Community Clinical Services!
Position: Social Worker BS Level (Behavioral Case Management Department)
Community Clinical Services, affiliated with St. Mary’s Health System, is seeking an organized, self-motivated person to provide case management services to children with emotional or psychiatric diagnoses as part of the Behavioral Health Home Organization. The BHHO is a team approach to case management and provides improved care coordination and access to services that address both mental and physical health, as a whole person approach to wellness. This person will function as part of an interdisciplinary team and will be responsible for managing a caseload of patients.
Responsibilities:
Provides community case management services for children/adolescents
Collaborates with other providers to provide holistic care to clients
Documents biopsychosocial assessment, progress notes and treatment goals in an Electronic Medical Record (EMR)
Benefits:
$500 Training/ conference funds provided annually (licensure renewal fees, examination fees, professional book reimbursement, etc. available)
Additional onsite trainings available (contact hours provided)
Five days of training allowable without the use of earned time

Clinical supervision provided, frequency dependent upon licensure requirements
Monthly case consultation with BHH team members
Education and Requirements:
Must be licensed as a Social Worker in the State of Maine (LSW or LSX)
1-3 years experience in children’s case management preferred
Ability to work in a team setting and autonomously
Excellent written, verbal and interpersonal skills required

Bilingual required, Somali speaking preferred
