By submitting this form, I certify that 1) I have reviewed the checklist provided in the University’s guidance “Export Compliance and Travel with Portable Electronic Devices” and meet all requirements necessary to utilize this Temporary Export Certification; and 2) to the best of my knowledge and belief, all of the information on this form is correct. I also understand that failure to report completely and accurately may result in export control and/or sanctions violations punishable by criminal or civil penalties. If any of the responses to these questions change, I will contact ORC regarding export compliance.
