Request Letter of Rec

Please follow this link to request a letter of recommendation (LOR) from Dr. Blossom via REDCap. Additionally, students should email Dr. Blossom before filling out the REDCap survey.

In order to write your letter I will need the information below at least 8 weeks before the letter due date.  Submission of this complete form starts the 8 week countdown. I do my best to keep track and on top of LOR requests and I receive a lotit is your responsibility to monitor LOR submission and due dates. If a due date is approaching (e.g., within 1 week) and I have not submitted your LOR yet, please feel free to check-in/remind me via email.
Please complete this form only once, making sure that you have all the requested information available before starting. Please note: LOR requests are deleted once they are completed (i.e., all LORs submitted during a single application cycle).
Required:
1. Submission instructions & due date(s).
2. Your CV/resume.
3.  A statement regarding your goals for grad school, your career, and why you are choosing this program specifically (this can be an early draft of your personal statement).
4. A Microsoft Excel document that includes (at a minimum) the following information for all of your applications organized by due date (earliest listed first): Due date, Full school name, Full program name (including degree type).
Optional:
1. Copy of your transcript (if you feel comfortable sharing; allows me to talk about your strengths as a student more broadly); please indicate whether you are comfortable with me sharing your grades.
2. A description of any specific work experience that you think is particularly relevant to the program to which you are applying (e.g., this might be an extension of something written on your CV).
3. If you took a class with me: You need to let me know whether or not you are comfortable with me sharing your performance (i.e., letter grade) in my class(es) in the letter.
As a general rule, my LORs are masked, meaning I share them directly with the school but not with the student.
