The goal of “What Matters Most” is to empower the older adult to advocate for what truly matters most to them as a unique individual, to be able to ask healthcare professionals all of their questions at each visit, and to talk with their loved ones and healthcare providers about their end-of-life wishes.

Conversation Project

The Institute for Healthcare Improvement has developed an initiative called the Conversation Project to help facilitate conversations with family and providers about end of life care preferences. The website provides helpful guides for clarifying your own thinking about end of life care, how to approach conversations with family and medical providers, and also provides more in-depth information on topics such as setting up a healthcare proxy. There are also specific resources for caregivers of individuals with Alzheimer’s disease and other dementias.
