Important Submission Guidelines

Access may only be requested by Faculty & Staff and must be approved by their direct supervisor.
Please do not submit a request on your own behalf—such requests will be denied.
Entering MaineStreet ID numbers anywhere on this form violates FERPA regulations – please use email addresses instead.
Submitting through this form is the fastest way to completion as requests sent outside the form are not queued.
Please consolidate all related access needs into a single request whenever possible.
