Change of Sequence Form
Change of sequence requests should be discussed with a student’s advisor first, and approved by the MSW Coordinator. Please contact Jennifer Crittenden, MSW Coordinator, to submit a change of sequence form. Jen will create a copy of this Google Doc for the advisor and student to fill out together. This copy will be placed in the student’s file for future reference and planning purposes.
Graduation Information
Graduation from the program involves two key steps. First, applying to graduate in MaineStreet. Second, submitting a completed Program of Study form. Deadlines vary by semester and are communicated by the Grad Coordinator and Internship Faculty Liaison to those who are graduating.
Once the Program of Study form is received, it is reviewed by the MSW Coordinator and submitted to the Graduate School for certification.

Licensure Verification Letters
If a student would like to sit for the ASWB licensure exam prior to graduation, and in the final semester of their program, they need to be in good academic standing and submit a program of study form using this link.
