Immigration Intake Form Instructions

  1. All incoming international students must complete this form.
  2. If you will be pursuing your program fully online, complete and submit this form. You do not need to complete the following Financial Documentation form.
  3. If you will be studying in-person, complete this form and the Financial Documentation Form.
  • Online students. (no documents required)
  • In-person students. (a legible copy of your passport’s biographical page–including a complete machine-readable section at the bottom–ready to upload; without, we can’t issue your I-20 or DS-2019)

All international applicants requesting an I-20 form (Certificate of Eligibility for F-1 Status) or DS-2019 (Certificate of Eligibility for Exchange Visitor J-1 Status) must demonstrate evidence of financial ability to support their education and living expenses while at the University of Maine or University of Maine Machias. Please follow the instructions below for completing this form.

  1. Bank statements, scholarship letters, or athletic agreements to verify sufficient funding
  2. Declaration of Finances form with required signatures from all non-university individuals or entities providing funds
  1. Determine the amount you need to show from your personal or sponsor’s finances. Click here to read a breakdown of the current costs related to your studies at UMaine or UMaine Machias. If you are bringing dependents with you, add $450/month for your first dependent and $150/month for each additional dependent.
  2. Gather the required financial documents (e.g., bank statements, scholarship award letters, etc.) you will use to document your funding sources. Acceptable types of documents are listed in the following section. All supporting documentation must be dated within 1 year of the submission date.
  3. If all or a portion of your expenses will be paid using personal or sponsor funds, you must complete the Declaration of Finances form, including all relevant signatures. All signatures must either be done in pen or with a verified digital signature. Unfortunately, we do not accept typed-in signatures.
  4. Fill in your financial information on the Financial Documentation e-form in e-OIP.
  5. Upload all applicable documents (e.g., bank statements, scholarship letters, Declaration of Finances form linked below, etc.) to the e-form.
  • Personal Savings. (requires official bank letter/transaction statement)
  • Family/Relative Personal Sponsor(s). (requires official bank letter/transaction statement per sponsor, who must also sign verification statements on the Declaration of Finances form linked above)
  • Scholarship. (requires official scholarship award notice from the institution/organization awarding the scholarship and must contain the applicant’s name, amount of money available for each year of study, duration of award, and the name “University of Maine” as the institution to which the award is applicable)
  • Loan. (requires official letter from the lending institution indicating approval of the loan and the amount approved)
  • Government/Employer/Other Organization. (requires official letter indicating the amount of support containing the applicant’s name, amount of money available for each year of study, duration of award, and the name “University of Maine” as the institution to which the award is applicable.

Disclaimer: Documents issued in the name of another college or university are not acceptable. The I-20 form will not be issued if documentation is incomplete or missing. Applicants must demonstrate financial support equal to or greater than the cost of attendance indicated by the link above.

Students will receive a bill once they have confirmed attendance and have been enrolled in classes for the upcoming semester. If you would like more information on your payment options, including how to make international payments please visit the Student Business Services website.
