POSITION SUMMARY:
Provides case management activities, education, and rehabilitative counseling to adult and adolescent behavioral health patients within the New England Eating Disorders Program at Sweetser.

ESSENTIAL FUNCTIONS:
Documents relevant biopsychosocial information from the patient and natural supports, as necessary, utilizing appropriate interview techniques.
Corroborates information from secondary sources regarding patient’s eating disorder history and biopsychosocial history.
Interprets assessment data to determine biopsychosocial and behavioral treatment/continuing care needs.
Assesses the outcomes of treatment interventions to determine on-going treatment needs.
Identify concrete service needs and provide integrated treatment with the client’s natural supports and providers to demonstrate best-practice
Develop short and long term goals for treatment with the individual and natural supports
Engages the patient in treatment.
Individualizes counseling in accordance with cultural, gender, spiritual, and sexual orientation differences.
Conducts individual, group and family sessions utilizing appropriate counseling knowledge, skills, and interventions to facilitate and assist in their recovery process.
Coordinates patient care services with staff and other services providers as needed.
Communicates the rationale of case management activities to the patient and natural supports when appropriate
Presents information about eating disorder information, services, and resources to the patient and natural supports when appropriate.
Responsible for all assessment documentation to include: diagnosis, assessment, mental status, level of care, progress notes, releases etc within the Electronic Health Record.
Coordinates and implements individual and group sessions.
Organizes and prioritizes workload, recognizing own needs and program needs.

 Report accidents and injuries in a timely fashion, demonstrates safe housekeeping practices, notifies supervisor of
potential health and safety concerns immediately, participates in health and safety training as required, understands and follows all Sweetser health and safety policies.
 Ability to support and recognize the important role volunteers play. Sweetser employees work with, support, collaborate with volunteers, and promote opportunities for volunteer engagement.
 Follow Suicide safer care practices.
 Perform all other duties as assigned.

EDUCATION:
 Master’s Degree is required.
CERTIFICATES, LICENSES, REGISTRATIONS REQUIRED:
 LCPC, LCSW, Nurse CS, Psy.D, or Ph.D
 Must maintain a valid state driver's license.
EXPERIENCE:
 Minimum of three (3) years of behavioral health counseling experience required.
KNOWLEDGE AND SKILLS:
 Must have the discipline and flexibility to operate within the team based framework of treatment.
 Excellent interpersonal skills are required, as the incumbent must interact with team members and patients in group
and individual settings
 Must possess a high degree of analytical and problem solving skills as the incumbent assesses patient condition,
motivation and ability, weighs multiple medical and lifestyle factors to formulate an effective treatment program.
