Fish River Rural Health (FRRH) is seeking a full-time Licensed Clinical Social Worker to join our patient-centered medical home. LCSW will work with patients in providing behavioral health and substance use disorder counseling services. The practitioner in this role will collaborate in an integrated model of care with Primary Care Providers to recognize, treat and manage behavioral health and psychosocial issues while acting as a contributing member to the Integrated Primary Care Team. A genuine interest to primarily provide high quality care to adolescent population. Competitive full-time benefit package, including health insurance and paid time off.
Minimum Qualifications Required: Must be a Licensed Clinical Social Worker with the State of Maine. The candidate must also possess strong communications skills, ability to work in primary care as part of an integrated team, ability and means to travel between service delivery sites. Previous FQHC experience a plus and candidate must be able to successfully pass a criminal background and DHHS background check.
