Social Workers
Dartmouth-Hitchcock has multiple full time (40 hours) open positions for MSW Social Workers in the Care Management department in Lebanon, NH. The hours are typically 8 AM – 5 PM with the occasional on call coverage, as well as weekends and holidays.
The Social Workers at Dartmouth Hitchcock work as members of the Interdisciplinary teams to provide psychosocial support and advocacy to patients and families in an effort to overcome barriers to care. They collaborate with a variety of community agencies to facilitate access to services and resources through NH and VT.

·      Outpatient Primary Care Pediatrics – Req #18000405
·      Outpatient Child Development/Child Psych – autism focused work with collaboration with schools – Req #180003RQ
Responsibilities:
·      Provides complete psychosocial assessments of patients and families.
·      Provides counseling to individuals and families to help solve social and emotional problems.
·      Coordinates community resources to address/meet identified needs of varied patient populations.
·      Assists families in adapting to difficult and complicated dispositions involving complex/catastrophic care needs, multiple agencies/providers and linkage to resources outside the community.
·      Provides consultation, assessment, referral and coordination in cases of suspected child or dependent adult abuse or neglect, or other forms of domestic violence.
·      Documents appropriate objective, concise and pertinent clinical data in the medical record.
·      Performs other duties as assigned.
Qualifications:
·      Master’s Degree in Social Work (MSW) required.
·      Licensed Independent Clinical Social Worker (LICSW) in New Hampshire preferred.
·      Social work experience in an acute, home care, or ambulatory setting preferred.
·      Strong organizational skills.
·      Good verbal and written communications skills.
·      Sound decision-making and judgment.
·      Time management skills.
·      Negotiation skills.
·      Proven ability to deal with conflict resolution.
·      Comfortable with basic computer skills.
