Social Work position

Lovejoy Health Center (Albion, near Waterville, Maine), a practice of HealthReach Community Health Centers is seeking a clinical social worker.
Our Family Practice team is comprised of physicians, nurse practitioners (family and psychiatric mental health), physician assistants, clinical social workers, care managers, as well as clinical and administrative support staff.
Opening: Explore our opportunity for a Licensed Clinical Social Worker to join our social worker and medical team in an integrated primary care practice.
Work side-by-side with medical providers serving 4,000 area residents. Our social workers offer consultation services to patients and primary care providers concerning behavioral changes to help patients achieve optimum wellness.
  • Engage patients in improving their wellbeing
  • Utilize solution-focused, short-term interventions
  • Serve people of all ages and economic situations in a Family Practice environment
  • Develop a common treatment plan with medical providers within a shared EMR addressing both physical and behavioral health needs
Requirements: LCSW required ~ LADC preferred
