Franklin Community Health Network is seeking a Licensed Clinical Social Worker in Farmington. The Licensed Clinical Social Worker will be part of a collaborative team that provides integrated mental health care to patients and families. The candidate will have experience in systems based family therapy or relevant modalities.
If hired, you may be eligible to apply for educational loan repayment through the HRSA National Health Services Corps.
The LCSW has comprehensive clinical responsibility for the clients assigned to him/her. These clinical duties will include:
  • Conducting and documenting individual and family psychosocial assessments
  • Documenting the findings of evaluations and progress in clients’ medical records
  • Conducting and documenting family and/or couples treatment of clients and their families
  • Maintaining contact with community resources for after care and providing comprehensive discharge planning services
Qualifications:
  • Maine State licensure to practice as a LCSW.
