This form must be submitted prior to key retrieval. The key holder themselves must come to Service Building A Room 103 between 7AM and 3PM to sign for the key (unless a special arrangement has been made). Please note, the Lock Shop will no longer be delivering keys. Please bring your University ID card with you. Prior to filling out this form, you will need the ID numbers of all the key holders. Once the key has been cut, the key holder will have 30 days to pick up the key.
Other types of keys (i.e. file cabinet, desk, safe, etc) should be requested through our work service request form. This form is for building access only.

Potential Cost
In order to stress the importance of key safety, we have broken down a potential cost for lost or stolen keys. A misplaced key could create a great security risk as we do not know who will come into possession of said key. If a key is lost or the department fails to return the key, they will be responsible for re-keying the area affected by the lost key. The re-key rate per door is $80. If one room has 2 doors, it will be $160 to re-key that single room. If the key opens multiple rooms, we have to factor in each door that must be re-keyed. By signing, you are agreeing to the potential cost of re-keying an area if the key is to become lost or stolen.
