Programs of Study

Doctoral

  • Those holding a master’s degree should submit the Program of Study before the end of the first year of study.
  • Those holding only a baccalaureate degree should submit the Program of Study by the completion of 12 credit hours or by the third registration, whichever comes first.

Master’s and Certificate of Advanced Studies 

  • Submit the Program of Study before the completion of twelve credit hours or by the third registration, whichever comes first.

Graduate Certificate

  • Submit the Program of Study before the completion of twelve credit hours or by the third registration, whichever comes first.

Changes to Your Program of Study

Change to Course Delivery

This form is to be used to change the method of course delivery for programs with an online option only (e.g. changing from an online to an on-campus student).

Notice of Comprehensive Examination

Doctoral students must submit this form within one week of examination.

Exception to Regulation and Appeal Process

Exception to Regulation

Students have the privilege of petitioning for exceptions to the regulations of the Graduate Faculty but must submit convincing evidence that the exception is needed and is warranted.  The Exception to Regulation form should be completed and submitted to us with appropriate signatures.

Appeal Procedure

A student filing an academic appeal is encouraged to request that the advisor or other faculty member choice act as a counselor and/or representative.

On-Leave and Withdrawal

On-Leave

Degree students not able to keep active status must apply for on-leave status.  Please refer to the policy found in the Policies and Regulations for more information.   Complete the On-Leave form and return to us for signatures.

Withdrawing from the University

To ensure proper posting of their academic and financial records, students who withdraw from graduate study should submit the Withdrawal form.

Submitting a Readmission Application

Students who have broken enrollment must apply for readmission before being allowed to continue working toward the degree.  The Readmission Application must accompany the $65 application fee.
