Additionally, students should email Dr. Blossom before filling out the REDCap survey.<\/p>\n<div><em>In order to write your letter I will need the information below\u00a0at least\u00a08 weeks before the letter due date.\u00a0 Submission of this complete form\u00a0<\/em><em><strong>starts<\/strong>\u00a0<\/em><em>the 8 week countdown. I do my best to keep track and on top of LOR requests and I receive <\/em><strong><em>a lot<\/em><\/strong><em> &#8211; <span style=\"text-decoration: underline\">it is\u00a0your responsibility to monitor LOR submission and due dates<\/span>. If a due date is approaching (e.g., within 1 week) and I have not submitted your LOR yet, please feel free to check-in\/remind me via email.<\/em><\/div>\n<div><\/div>\n<div><strong>Please complete this form only <span style=\"text-decoration: underline\">once<\/span>, making sure that you have all the requested information available before starting.\u00a0<em>Please note<\/em>:<\/strong>\u00a0LOR requests are deleted once they are completed (i.e., all LORs submitted during a single application cycle).<\/div>\n<div><\/div>\n<div><strong>Required:<\/strong><\/div>\n<div>1. Submission instructions &amp; due date(s).<\/div>\n<div>2. Your CV\/resume.<\/div>\n<div>3.\u00a0 A statement regarding your goals for grad school, your career, and why you are choosing this program specifically (this can be an early draft of your personal statement).<\/div>\n<div>4. A Microsoft Excel document that includes (at a minimum) the following information for all of your applications organized by due date (earliest listed first): Due date, Full school name, Full program name (including degree type).<\/div>\n<div><\/div>\n<div><strong>Optional:<\/strong><\/div>\n<div>1. Copy of your transcript (if you feel comfortable sharing; allows me to talk about your strengths as a student more broadly); please indicate whether you are comfortable with me sharing your grades.<\/div>\n<div>2.
