Example of Curriculum/Program Change Packet

This is an example of how Curriculum Changes and Program Changes should come to UPCC:  Approved BS in Computer Science 2013

It should always included:
1) A cover letter with lines for those to sign and date who need to approve the changes (Leader, Initiating Department/Unit(s); College(s) Curriculum Committee Chair(s); Dean(s); Associate Provost and Dean for Undergraduate Education)

2) Memo clearly stating what the changes are and the reasons for the change. Should also include WHEN it will be effective.

3) The packet should include the PREVIOUS curriculum, and the NEWLY PROPOSED curriculum so the committee can easily review the proposed changes.

4) It should be accompanied by any New Courses or Modifications to existing courses using the New Course/Mod form: New Course-Mod Form2-2013
