So You Want to Farm in Maine Impact Evaluation 2013
You are invited to participate in a voluntary evaluation of the So You Want to Farm in Maine webinar series being conducted by Kathy Hopkins and Donna Coffin of the University of Maine Cooperative Extension. The purpose of this evaluation is to understand if this webinar series results in any change or impact for program participants. The information provided will be used to tell the funders of this program, the University of Maine Cooperative Extension about the impacts of the webinar series. A summary of the data will be presented at an annual meeting. The summary will also be used to help in planning future programs.
What Will You Be Asked to Do?
If you decide to participate, you will be asked to answer questions on an online survey that will be available through the secure website, Constant Contact, and should take about 10 – 15 minutes.
Except for your time and inconvenience, there are no risks to you from participating in this survey.
While this study may have no direct benefit to you, this survey will help us learn more about the impacts of our programming.
Your name will not be connected with your responses; responses are anonymous. Data from this survey will be kept in a password-protected file for one year and then will be deleted.
You don’t have to answer any question you don’t want to, and you can end the survey at any time.
For more questions about this survey contact Kathy Hopkins at 1-800-287-1495 (in Maine) or 207-474-9622. If you have questions about your rights as a research subject, contact Gayle Jones, Assistant to the University of Maine’s Protection of Human Subjects Review Board, (207) 581-1498 or email email@example.com.
