DISCLOSURE—PREPARATION OF A CONSUMER REPORT
To process your employment application with the University of Maine, an investigative consumer report (background check) may be conducted by Verified Credentials, Inc. In accordance with the U.S. Fair Credit Reporting Act § 606, we notify you of the following: A background check report may contain information bearing on your character, general reputation, personal characteristics, mode of living and credit standing. Information may include, but is not limited to: employment history, education, criminal records, credit history, motor vehicle records, personal references, and any data provided on this application, or during the interview process.
You have the right to request a complete and accurate disclosure of the nature and scope of the investigation requested. This disclosure shall be made in a writing mailed, or otherwise delivered, to you not later than five days after the date on which your request for such disclosure was received from you or the report was first requested, whichever is later. You also have the right to request a written summary of your rights under the Fair Credit Reporting Act.
