• Member Change In Status Form - To be completed each semester and turned into the Graduate Assistant for Fraternity & Sorority Affairs.

• Chapter Officer Change Form - To be completed each semester and turned into the Graduate Assistant for Fraternity & Sorority Affairs.

• Monthly Community Service Form (Excel) – To be completed each semester and turned into the Graduate Assistant for Fraternity & Sorority Affairs.

New Member Bid Acceptance forms must be signed in the Office of Fraternity & Sorority within 72 hours of a new member accepting a bid.
