Clinical Site Evaluation Form
As clinical education is such an important aspect of any athletic training education program, students are placed in clinical experiences consistent with coursework. You will be graded within these experiences on proficiency of skills, ability to problem solve and the ability to perform professionally through written and practical testing as well as by timely evaluations. You will be evaluated 2 times per clinical experience. The first will be completed by the ACI at mid-experience while the second will come at the end of the experience. Attached is an example of the ATS Evaluation for KPE 201 Athletic Training Clinical Skills – 1. This Evaluation will be completed by the ACI, reviewed with you and then sent to the Program Director who will also meet with you to discuss the outcome.
You too, will have the opportunity to evaluate the site of your clinical experience and your ACI. A copy of each evaluation has also been attached for your viewing. These will be completed at the end of each clinical experience and will be shared with the Program Director and the ACI if appropriate. If at any time, you are unhappy with your clinical experience or with your ACI, please communicate this with your Program Director immediately so we can help to create a healthy learning environment.
